This is a 12 year old Glenlivet single malt, bottled under the Smith's name at 40% ABV in a 75cl bottle. It was distilled in May 1964 and bottled in October 1976.
Glenlivet began as a farm called Minmore, which George Smith bought in 1858 before setting up a distillery on the site. The distillery started production in 1859, using equipment from two closed distilleries, Cairngorm and Upper Drummin. It is now owned by Pernod Ricard.
