This is a Balvenie 12 Year Old Single Barrel, number 765, a Speyside single malt bottled at 47.8% ABV in a 70cl bottle. It comes from one single barrel.
Balvenie takes its name from nearby Balvenie Castle and stands in Dufftown, often called the unofficial whisky capital of Speyside. William Grant & Sons built the distillery in 1892 from the remains of the castle's ruined guest house. It has eight stills today, and its water comes from the Robbie Dubh spring, the same source that supplies Glenfiddich.
